On July 30, TransForm's board of directors held its annual general meeting and it was an opportunity to reflect on the successes of the past year. New Ecosystem Cultivates Innovation in Erie St. Clair

With an aging population, an explosion in technology options and a need to serve patients most efficiently, the healthcare sector is in the midst of a transformation. In June, health service providers across the Erie St. Clair region announced a joint partnership to form an innovation ecosystem. The new ecosystem will support these changes and drive the need to be more innovative and collaborative in providing services, programs and systems.
Clinical Savings Top $3 Million this Fiscal Year
TransForm continues to make strides in identifying and assisting member hospitals implement more than $3 million in annual supply chain cost savings. A significant portion of these savings is the $2 million achieved through the value analysis team, consisting of TransForm supply chain and hospital employees.  Read More. 
TransForm Seeking Customer Feedback
As part of TransForm's commitment to delivering exceptional service and enhancing the customer experience,  a customer feedback survey will be launched this fall. TransForm will specifically measure stakeholder leadership perceptions and experiences of customer services levels. The results will be used by TransForm's leadership to develop an action plan for service enhancements within each respective department. More information, including access to the survey will be released in a formal communique on each hospital's intranet site. 
All Hospitals Now Connected to EHR

As of July 16, 2015, 100 per cent of the 67 acute care hospital sites in south west Ontario (SWO) now provide data as part of the integrated electronic health record (EHR) for Ontarians.


 

Data from the 67 acute care hospital sites in south west Ontario is now accessible in the cSWO Regional Clinical Viewer, ClinicalConnectTM, following the successful integration of the remaining six hospital sites in the Erie St. Clair Local Health Integration Network (ESC LHIN). This milestone represents significant robustness of the integrated EHR, as well as achievement of the cSWO Program's acute hospital integration target.

Member Hospital Project Spotlight: CVIS Launch
A major project that was more than a year in the making went live at Windsor Regional Hospital (WRH) on June 2, 2015. The integrated Cardiovascular Information System (CVIS) at WRH Ouellette, is a single database that houses the complete cardiovascular electronic medical record, including nurse charting, images and reports for cardio patients at both WRH campuses. It also introduced new electronic charting and reporting capabilities for Interventional Radiology. Read More.  
Operational Dashboard to Increase Visibility and Accountability
TransForm recognizes the importance of clear, accountable and transparent reporting to its member hospitals. We have taken an approach of continuous improvement in how we provide our performance metrics in a way to best meet hospital needs. A new enhancement to our performance metrics will launch this summer that will make it easier for member hospitals to visualize and track performance, as well as have the ability to drill-down into many of the metrics to better understand the "why" of the metric results.  Read More.

Both the standards and philosophy will guide organizational service improvement initiatives through a model of continuous quality improvement and help TransForm embrace a culture of service excellence.

Linda received a token of appreciation from Windsor Regional Hospital for going above and beyond her regular duties to assist the hospital's human resources department. As part of the hospital's recognition program, Linda's name will appear on the hospital's honour roll at  www.wrh.on.ca.
 
"I felt compelled to communicate my gratitude for the work that Edena Shreve has done related to data for the Systemic Treatment Funding Model. Briefly, we receive a report from CCO that tracks our volumes and their associated funding. When reviewing those early reports, it became obvious we were missing significant volumes. Edena really worked relentlessly to figure out how & where it was not flowing to and from and why not."

Wendy and Jane were recognized by employees at Windsor Regional Hospital (WRH) for their outstanding effort with the GUI flow chart project. 

"We completed a downtime with minimal patient impact, have developed flows that will better enhance patient data and patient safety. Without individuals like yourself who go beyond the job and focus on patient and stakeholders none of this would have been possible.  Again kudos to both of you for going above and beyond."
