What We Learned: HIN's third annual Managing Care Transitions survey captured efforts by 86 organizations to strive for Triple Aim goals of better care at improved cost during transitions of care. The survey measured existing and planned programs, targeted transitions and populations, transition team members and responsibilities, and much more. According to survey respondents, the top five training areas for care transition teams are:
- MI: 60 percent
- Health literacy: 46.7 percent
- Chronic DM: 40 percent
- Polypharmacy: 40 percent
- Other: 26.7 percent
