What We Learned: HIN's fourth annual industry survey on healthcare case management, administered in May 2013, captured data on case management responsibilities, diagnosis most responsive to case management, how individuals are identified for case management, and much more. According to the 118 healthcare companies who responded, the top five challenges of embedding a case manager are:
- Lack of CM peer support: 15 percent
- Staff buy-in: 15 percent
- Communication: 12.5 percent
- Reimbursement: 12.5 percent
- Patient engagement: 10 percent
