For Which Primary Diagnosis Is Case Management Most Effective? This Week's Challenge: According to 29 percent of healthcare companies who responded to HIN's third annual case management survey, diabetes is the top primary diagnosis for which case management is most effective. We wanted to see which other diagnoses case managers are effective at helping patients to manage.

What We Learned: HIN's Healthcare Case Management e-survey captured the efforts and contributions of case management from 153 healthcare organizations, with a concentration on three new areas: trends in embedded case management, clinical conditions targeted by case managers and preferred case management certifications. According to survey respondents, the top five diagnoses for which case management is most effective are:

  • Diabetes: 29 percent

  • Behavioral health: 11 percent

  • CHF: 11 percent

  • Cancer: 11 percent

  • CVD: 9 percent
