What's the Greatest Challenge Posed by a PHM Approach? This Week's Challenge: According to nearly 43 percent of respondents to HIN's first annual survey on Population Health Management (PHM), the greatest challenge posed by a PHM approach is patient engagement. We wanted to see what other challenges exist for PHM programs.

What We Learned: HIN's PHM survey, conducted in September 2012, documented the efforts of 102 healthcare organizations in PHM, including program areas, populations served, how the appropriate level of intervention is identified and much more. According to the companies who responded to the survey, besides patient engagement, the top challenges posed by a PHM approach are:

  • Patient engagement: 42.4 percent

  • Provider engagement: 21.2 percent

  • Reimbursement: 12.1 percent

  • Other: 9.1 percent

  • Performance measurement: 6.1 percent

  • Technology: 3 percent

  • Care access: 3 percent

  • Risk assessment: 3 percent
