Top 5 Ways to Identify Individuals for Case Management This Week's Challenge: Care coordination by healthcare case managers is helping to drive clinical and financial outcomes in population health management and bolster emerging models of care such as the patient-centered medical home and the accountable care organization. We wanted to see how individuals are identified for case management.

What We Learned: HIN's fourth annual industry survey on healthcare case management, administered in May 2013, captured data on case management responsibilities, diagnosis most responsive to case management, challenges of embedded case management, and much more. According to the 118 healthcare companies who responded to the survey, the top five ways individuals are identified for case management are:

  • Case management assessment: 62.2 percent

  • Provider referral: 56.7 percent

  • Claims data: 50 percent

  • Hospital census: 47.8 percent

  • Self/family referral: 40 percent
