Who Coordinates Care Transitions?

Who Coordinates Care Transitions? This Week's Challenge: Proper management of transitions in care — the handover of an individual's care from one health setting to another — has the potential to dramatically hasten that person's return to health, as well as reduce the likelihood of a return ER visit or rehospitalization. We wanted to see which healthcare professionals coordinate care transitions.

What We Learned: HIN's third annual Managing Care Transitions survey captured efforts by 86 organizations to strive for Triple Aim goals of better care and better care at improved cost during transitions of care. The survey measured existing and planned programs, targeted transitions and populations, transition team members and responsibilities, and much more. According to survey respondents, the top five healthcare professionals who coordinate care transitions are:

  • Case managers: 35.3 percent

  • Other: 29.4 percent

  • Social workers: 29.4 percent

  • Transition coaches: 29.4 percent

  • Discharge planners: 23.5 percent
